Box 7: Summary of Progestogen only Implant consultation/procedure

Discuss all contraception options
Medical eligibility
Avoid if:
  • On liver enzyme inducers
  • History of breast cancer
  • Unexplained vaginal bleeding
  • Develop CVD/stroke while using POIm
  • Significant liver disease
Discuss possible side effects
  • Altered bleeding (often persistent)
  • Weight gain
  • Headaches
  • Acne
  • Reduced libido
  • Skin atrophy
Explain procedure

Rule out pregnancy

Consent
Extra precautions needed
  • If starting within 5 days of cycle none needed
  • After 5 days, extra precautions for 7 days
  • If switching from POP, CHC or IUC extra precautions for 7 days*
Insert & document
  • Only trained professional to insert
  • Insert 8-10cm from medial epicondyle, sterile conditions
  • Document site, batch number and expiry date
Follow-up advice
  • Keep dry for at least 24hours
  • Review if problems
  • Date when POIm needs to be changed (3 years)

*Up to day 5 only applies to true cycles.  These rules do not apply for women on CHC.  For women on CHC, 7 days extra precautions are needed if inserted from day 3 of HFI and during first week of taking CHC.  If POIm delivered week 2 or 3 then no extra precautions needed.
